HSJ and Doccla: Webinar Highlights
Doccla, in partnership with HSJ, hosted a webinar exploring a pressing question facing the NHS today: how do we transform care so it is closer to home, preventative, and more person-centred?
The session, chaired by Ben Clover (Bureau Chief, HSJ), brought together a panel of leaders shaping the future of care delivery:

Aligning with the 10-Year Plan
The recently published NHS 10-Year Plan sets out three defining shifts:
- From hospital to community
- From analogue to digital
- From sickness to prevention

Virtual Care in Action
The panel highlighted how Doccla’s proactive model of care is already addressing some of the system’s toughest challenges:
- Scalable Virtual Wards that free up hospital beds while keeping patients safe at home.
- Proactive long-term condition support that reduces unplanned admissions and enables earlier intervention.
- Integrated neighbourhood models, where multidisciplinary teams work seamlessly across primary, community, and specialist care to support the person, not just the episode.
Real-world programmes are already showing tangible results:
- In Leicester, proactive frailty monitoring delivered an 89% reduction in GP appointments and a 61% reduction in bed days.
- In Bristol, the Living Well with COPD programme achieved a 34% reduction in non-elective admissions, alongside patient feedback where 77% reported improved management of their overall health.
- In Hertfordshire, the Hospital at Home service has cared for around 23,500 patients since 2022, avoiding an estimated 1,500 frailty admissions each year. During peak winter pressures, 290 patients were supported at home — more than half the inpatient capacity of Lister Hospital. The model is also playing a crucial role in urgent care, with around 25% of referrals coming directly from 999 and A&E calls. Care is delivered at a fraction of the cost (£85 per bed day vs £450 in hospital) and consistently achieves 90%+ positive patient feedback

Alongside individual stories, services like Hertfordshire’s report over 90% satisfaction across thousands of patients cared for at home.
