Doccla and SWL ICB Partner to Deliver the Next Generation of Virtual Care
We’re proud to announce our partnership with South West London Integrated Care Board (SWL ICB) to deliver integrated Virtual Remote Monitoring across both Virtual Wards and Care Homes for the next two years. This landmark project will help scale up high-quality, digitally enabled care for a population of approximately 1.5 million people.
A System-Wide Vision for Virtual Care
SWL ICB brings together six local authorities, 350 care homes and a network of GP practices, community teams and hospital providers.
Doccla’s role will be to deliver an end-to-end remote monitoring service that supports both:
- Virtual wards: enabling early discharge and at-home care for patients with conditions such as frailty, respiratory illness and heart failure
- Care homes: supporting care staff to monitor residents proactively, identify early deterioration and reduce avoidable hospitalisations with automated NEWS2 scoring.
As part of this partnership, Doccla will provide a full end-to-end service, including:
- Software access: The Doccla Patient App, Carer App, and Clinician Dashboard to support safe, user-friendly remote care across settings.
- EHR integration: Live connectivity with EMIS, PCS, Nourish, and Oracle Health (Cerner), ensuring seamless data flows across virtual wards and care homes.
- Device provision and logistics: Passive multi-parameter wearables, Bluetooth-integrated devices, SIM-enabled tablets, and managed delivery and collection from hospital sites and patients’ homes.
This partnership continues to directly support all three core shifts outlined in the NHS 10 Year Plan: moving care from hospital to community, from analogue to digital, and from sickness to prevention. As we continue to scale virtual care across the NHS, these shifts guide how we deliver smarter, more sustainable services supporting people to stay well at home, improving early intervention through technology, and expanding equitable access to proactive and preventative care.
Seamless Integration, Real-Time Insights
Doccla’s platform is built to support frontline teams with real-time insights, seamless integration and safe, scalable remote care. In South West London, we aim to:
- Reduce response times through real-time dashboards and automated alerts that help clinicians detect deterioration earlier
- Ensure continuity of care by integrating with systems used across virtual wards and care homes
- Simplify clinician workflows by streamlining data flows between the CRM hub and four local virtual wards
- Support tailored care delivery through flexible device provisioning that aligns with site-specific pathways and populations
