Without this consent, we can’t complete your onboarding or send you monitoring devices.
✅ Consent Requirements
- I agree to use the service and accept the Data Privacy Policy.
- I will read the Patient Information Leaflet and any instructions on safe equipment use, and will contact Doccla Support if I have any questions.
- I agree that Doccla can receive and share relevant information with my healthcare team and other authorised professionals to provide quality care.
- I agree that Doccla may contact me about my health, including reminders if I miss my readings, and I will respond when they reach out.
- I understand that Doccla is not an emergency service.
- I understand that if I do not respond to calls or texts, Doccla or my healthcare team may contact my next of kin, arrange a home visit, or call emergency services if there are urgent concerns for my safety.
- I consent to my personal data being used and processed in line with the Data Privacy Policy and data protection laws.
- I confirm that I am at least 13 years old, or that I am giving consent on behalf of a child or someone who has authorised me (e.g. Power of Attorney).
- I understand I can withdraw my consent at any time by contacting Doccla Support.
✅ Important Explicit Consent
- I understand that Doccla requires my explicit consent to process my health data in compliance with data protection laws and if such consent is not provided by me, Doccla will be unable to provide its services to me. I consent to my health data being processed by Doccla.
