Before we set you up on the Doccla Virtual Care Platform, please review and confirm the points below, and also confirm you have read our Patient Privacy Notice. These confirmations help us provide your care safely.
Your Confirmations
- I wish to use the service and have read the Patient Privacy Notice.
- I will read the Patient Information Leaflet and any instructions on safe equipment use, and will contact Doccla Support if I have any questions.
- I acknowledge that Doccla can receive relevant information about me from, and share relevant information about me with, my healthcare team and other authorised professionals involved in my care (such as the hospital or trust treating me and my GP practice) in order to provide quality care.
- I understand that I have the right to object to this sharing of my information by contacting Doccla Support or emailing dpo@doccla.com. If I object, a Doccla clinician will discuss this with me. I understand that Doccla cannot provide the service without this sharing, so if I continue to object I will not be able to use (or continue to use) the service, but the rest of my NHS care will not be affected and will continue through my usual NHS team.
- I acknowledge that Doccla may contact me about my health, including reminders if I miss my readings, and I will respond when they reach out.
- I understand that Doccla is not an emergency service.
- I understand that if I do not respond to calls or texts, Doccla or my healthcare team may contact my next of kin, ask my GP or community team to check on me, or call emergency services if there are urgent concerns for my safety.
- I have read and understood how Doccla will use my personal data, as set out in the Patient Privacy Notice.
- I confirm that I have capacity to provide these acknowledgements and confirmations, or that I am providing them on behalf of a child I have parental responsibility for, or on behalf of an adult for whom I hold a registered Health and Welfare Lasting Power of Attorney or a court appointment covering health and welfare.
- I understand I can stop using the Doccla service at any time by contacting Doccla Support. If a trust referred me into the Doccla platform, they will be informed. Stopping the service does not delete records Doccla must keep by law.
