This independent analysis examines how an integrated approach - combining remote monitoring, soft signs questionnaires, virtual clinician support and coordinated escalation pathways - has reduced healthcare utilisation, with significant decreases in GP consultations (89%), emergency attendances (79%), ambulance callouts (71%) and inpatient bed days (up to 75%). The evaluation also highlights improved patient safety, confidence, and quality of life, alongside the potential for virtual care to deliver capacity-releasing savings and inform future system-wide deployment.
